Registered Nurses Home Health/Hospice

Registered Nurses for the Certified Home Health Agency and Certified Hospice Program. The Certified Home Health Agency RN responsibilities include short-term skilled nursing care to homebound residents with intermittent on call responsibilities for after hours coverage. The Hospice RN responsibilities include providing palliative care to hospice patients within the interdisciplinary model which includes on-call as needed.  Home Care or Hospice experience preferred, 2 years of Medical/Surgical nursing experience and excellent interpersonal communications skills are required. New York State license to practice as a Registered Professional Nurse and driver’s license are required.
