Casual licensed practical nurses Hospice/Home Health

Casual Licensed Practical Nurses for the Certified Hospice Program and Certified Home Health Agency are available.  The Hospice LPN responsibilities include providing palliative care to hospice patients within the interdisciplinary model which includes on-call as needed.  The Certified Home Health Agency LPN responsibilities include short-term skilled nursing care to homebound residents with intermittent on call responsibilities for after hours coverage.  New York State license to practice as a Licensed Practical Nurse and driver’s license are required.
